I'm not currently working in a hospital transfusion service but can imagine using type specific, Rh matched would require adequate inventory and enough reagent to screen. In order to anticipate the need and allow for staffing shortages blood would need to be screened proactively. THe alternative would be to make arrangements for the supplier to proactively screen and hold units--this may result in unfavorable delays for the patient.

This is quite an interesting case and especially so to Blood Bankers as the virus causing Fifth disease is Parvovirus B19. It is also transmitted through blood and derivitives. We test samples from plasma donations that will be sold for further manufacture into pooled lifesaving products for this virus. Due to the potential for plasma donation from asymptomatic individuals with extremely high viral loads contaminating large pools as well as the resistance of this virus to the physicochemical treatments used in plasma manufacture, and the lack of testing options, it has presented a significant challenge for both collection facilities, testing facilities and plasma manufacturers.

Approximately 18 months ago our hospital, Our Lady of the Lake Regional Medical Center (OLOL), Baton Rouge. LA, signed a Cooperative Endeavor Agreement (CEA) with Louisiana State University (LSU). This CEA will have far reaching impacts to our facility, our community and our state. In particular the CEA;
1. Esentially shuts down a state run hospital in Baton Rouge - Earl. K. Long Medical Center.
2. Transfers indigent patient care responsibility to OLOL.
3. Spurs a $180 million addition to OLOL which includes a Level 1 Trauma Center scheduled to open in Sept 2013.
4. Transfers all Graduate Medical Education for LSU students to OLOL.

As Director of Blood Donor Services @OLOL, it is my responsibility to plan and implement actions to provide for an adueate and safe blood supply for our repidly growing facility. No small task - for sure.

Good comments, it does seem like a high fequency and would be hard to get feedback to the employees that often. Seems yearly might be good with a structured feedback process...
